The sports medicine fellows are licensed physicians who are seeking additional training in the management of sports related injuries and conditions. As part of their clinical experience and education, they function as an extension of the OA Sports Medicine Center.

John P. Colianni, M.D.
Dr. Colianni is a graduate of Middlebury College, in Middlebury, VT. He received his medical degree from the University of Minnesota. Dr. Colianni completed his residency training at Maine Medical Center and is currently a Sports Medicine Fellow through the Department of Family Medicine in collaboration with OA Centers for Orthopaedics. He serves as the team physician for Gorham High School, and assists with St. Joseph's College and the Portland Pirates (AHL). Dr. Colianni is also a medical director for the Maine Marathon and provides coverage for the Lake Placid Ironman and Beach to Beacon events.

Elizabeth E. Rothe, M.D.
Dr. Rothe is a Maine native who grew up in Leeds, Maine. She attended the University of Pittsburgh for both her undergraduate studies and for medical school. She returned to Maine in 2005 to start a residency in Family Medicine at Maine Medical Center. She is currently a fellow in Sports Medicine through the Department of Family Medicine in collaboration with OA Centers for Orthopaedics. She enjoys the outdoors and spending time with her husband and her son, who was born in May, 2008.

Physician Assistants/Nurse Practitioners

Physician Assistants (PAs) and Nurse Practitioners (NPs) are licensed health care professionals who practice as part of your medical team in all areas of medicine, with physician supervision.

In most states, Maine included, PAs and NPs can treat patients and write most prescriptions when their supervising physician is away. This helps to maintain continuity of care when doctors are unavailable. They are trained to recognize when patients need the attention of a supervising physician or another specialist.

Their training is modeled after medical student training, to include anatomy, pharmacology, pathophysiology, clinical medicine, and physical diagnosis and treatment.  This is followed by clinical rotations.

How do physicians and PAs/NPs work together?
The relationship between a PA/NP and his/her supervising physician is that of mutual trust and respect; they function as a team. The PA/NP is a representative of the physician and treats patients in the style and manner developed and directed by the supervising physician.

What can they do?
Physician assistants and nurse practitioners provide a broad range of medical and surgical services, which have traditionally been performed by physicians. PAs and NPs are trained to conduct physical examinations, diagnose illnesses, order and interpret x-rays and laboratory studies, write prescriptions, assist in surgery, develop treatment plans, and instruct and counsel patients.  They also treat injuries by suturing, splinting, and casting. Additionally, PAs and NPs may assist in surgery. These providers may see patients independently and/or directly with a physician.

When might I see a PA/NP at OA?
At the time of:

  • Initial visit
  • Follow-up Care
  • Preoperative Visit
  • Emergency Room Visit
  • Cast Room Visit
  • Hospitalization
  • Surgery, in the Operating Room

You may see the physician and his/her PA/NP at the same visit. However, if you do not see the physician, please know that the physician assistant or nurse practitioner discusses and reviews your case with his/her supervising physician.

Will my insurance pay for me to see a PA?
It is usual and customary for insurance to cover services rendered by a physician assistant and nurse practitioner.

Chris T. Hillman, PA-C
After graduating from the University of Colorado at Boulder in 1978, Chris worked in women's health care for four years before attending the Duke University Physician Associate Program. She graduated in 1984 and joined OA's Sports Medicine Center. She presently works with Dr. Douglas Brown, focusing on the management of acute knee injuries. Over the years, Chris has also worked in OA's Spine Center. Chris is certified by the National Commission on Certification of Physician Assistants and is a member of AAPA and DEAPA.

Brian A. Lawhead, PA-C    "Andy"
Before participating in the Physician Assistant program at King's College, Andy received his Bachelor's Degree in Health Science from Lock Haven University in Pennsylvania. Since graduation, he has worked in orthopedics and joined OA in 1996. Andy supports the physicians in our Trauma and Fracture Center, working primarily with Dr. White. Andy is certified by the National Commission on Certification of Physician Assistants and is a member of AAPA, PAOS, and DEAPA.

Paul J. Evans, PA-C
Paul has been with OA since 1998. Paul supports our Trauma and Fracture Surgery Center, primarily assisting Dr. Babikian. He also assists Dr. Guay in the Spine Center. He has a Masters of Physician Assistant Studies from the University of Iowa, and is certified by the National Commission on Certification of Physician Assistants. He is also a member of the AAPA and PAOS.

Scott M. Benevides, PA-C
Scott earned his Physician Assistant degree from the University of New England in 2000. Prior to joining OA, he worked with general/vascular surgeons. He works in our Sports Medicine Center with Dr. Murray. Scott is a member of AAPA, PAOS, and DEAPA, and is certified by the National Commission on Certification of Physician Assistants.

Brian L. Halla,  PA-C
In 1987, Brian graduated from Duke University with a degree in Political Science. He then served in the US Navy as a Surface Warfare Officer and as a Middle East Specialist. Brian enrolled in the Physician Assistant program at the University of New England and graduated in 2002. Since joining OA in 2002, Brian has worked primarily with Dr. Eric Hoffman. Brian is certified by the National Commission on Certification of Physician Assistants.

Cheryl S. DeGrandpre, PA-C
Cheryl graduated from the University of New England in 2002, with a Master's Degree in Physician Assistant Studies. She worked in emergency medicine prior to coming to OA. Cheryl initially worked in the Sports Medicine Center and now works primarily with Dr. Camuso in the Trauma and Fracture Center. She loves all sports, and enjoys golf, cross country skiing, and spending time outdoors. She is certified by the National Commission on Certification of Physician Assistants and is a member of AAPA, DEAPA, and PAOS. Cheryl is active on the Board of Directors for DEAPA.

Kristina E. Kramer, MMS, PA-C
Kristina earned her bachelors degree in Sports Medicine from the University of North Carolina, Greensboro (UNCG) in 2001. She attended UNCG on a full merit scholarship and served as an athletic trainer for 3 years. She obtained her Masters of Medical Science Degree from the Wake Forest University School of Medicine in 2004. After completing her education, Kristina practiced dermatology in San Diego, California for two years before relocating to Maine to be near family. Kristina is certified by the National Commission on Certification of Physician Assistants and as an athletic trainer through the National Athletic Trainers Association. She works with Dr. Asherman specializing in foot and ankle surgery.

Michael R. Dore, PA-C
Michael earned a Bachelor of Science Degree in Business Administration from Husson College in 1993. After a few years, he furthered his education and graduated from the University of New England in 2003, with a Master's Degree in Physician Assistant Studies. He worked in emergency medicine and with the veteran population in an urgent care setting prior to coming to OA. Michael works with our physicians in the OA Joint Replacement Center. He is certified by the National Commission on Certification of Physician Assistants and is a member of the AAPA.

Rupinder Gill, PA-C
Rupi graduated with a degree in Biology from Siena College in upstate New York. Following that he assisted in cancer research at Maine Medical Center’s Research Institute. Rupi continued his educational pursuits by attending the Massachusetts College of Pharmacy. Ultimately, he enrolled in the Physician Assistant Program at the University of New England where in 2003 he received his Masters of Science in Physician Assistant Studies. Currently, Rupi subspecializes in shoulder reconstructive surgery, with Dr. Endrizzi, and helps to provide orthopaedic emergency and trauma coverage in Portland. Rupi is certified by the National Commission on Certification of Physician Assistants and holds membership in the AAPA, DEAPA, and PAOS.

Diana Brennan, PA-C
Diana graduated from Hahnemann University's Physician Assistant Program in 1985. After moving to Portland in 1986, she worked at OA with Dr. Omar Crothers for a number of years. After a hiatus to raise her family, she has returned to OA and is now working with Dr. John Chance in the Hand Center. Diana is certified by the National Commission on Certification of Physician Assistants and is a member of the AAPA and DEAPA.

Sarah C. Starbird, PA-C
Sarah graduated from the Duke University Physician Assistant Program in 1986. Since then, she has worked in hand surgery, family practice, and most recently in emergency medicine. She returns to hand surgery working primarily with Dr. Matthews and the Hand Center team. She is certified by the National Commission on Certification of Physician Assistants and is a member of AAPA.

Denise Emple, PA-C
Denise graduated from Northeastern University's Physician Assistant program in 1991. Prior to moving to Maine in 1992, she spent a year working with the General Surgery Department at Faulkner Hospital in Jamaica Plain, MA. She spent several years at OA working with Dr.'s Becker and Chance. After spending several years at home raising her 3 children, she returned to OA and now works with Dr. Brown in the Sport Medicine Center. Denise is certified by the National Commission on Certification of Physician Assistants, and is a member of the DEAPA and AAPA.

Cynthia J. Bolles, PA-C
Cindy graduated Cum Laude with an Exercise Health & Fitness/Education B.S. degree from Northeastern University. While at Northeastern, she trained with the national crew team and attended Olympic Development camps. She received her Physician Assistant degree from the University of Medicine and Dentistry of New Jersey in 1991. Since her graduation, Cindy has worked in general surgery, emergency medicine, orthopedics, and as a hospitalist. In addition to working part-time at OA, she keeps busy raising her five children, and is a competitive figure skating coach. She is certified by the National Commission on Certification of Physician Assistants and is a member of AAPA.

Bernadette H. Shaw, PA-C
Bernadette graduated from Brown University with a degree in Human Biology. She coordinated clinical research trials at Massachusetts General Hospital in the Center for Women's Mental Health before obtaining a Master of Medical Science degree from Yale University. For the past three years she has worked as an orthopedic physician assistant in the Greater Portland area providing care to patients in sports medicine, trauma, total joint replacement, spine and pediatrics. She enjoys competing in local triathlons, windsurfing and skiing. Bernadette is certified by the National Commission on Certification of Physician Assistants and is a member of AAPA and DEAPA.

Charles J. Roth, PA-C    "Chaz"
Chaz graduated from the University of New England Physician Assistant Program in 1998. Prior to his career as a physician assistant, he worked as an athletic trainer for over 15 years, primarily with high school athletes. Chaz has served on the Board of Directors for the Downeast Association of Physician Assistants and was a founding member and past president of the Maine Athletic Trainers Association. He has served as adjunct faculty for the University of New England Physician Assistant Program, and while at the University of New England served as class president. Chaz has significant experience in neurosurgery, orthopaedic surgery, sports medicine, and occupational medicine. He enjoys the challenges of the human spine and works with Dr. Michael Binette and the Spine Center Team. He is certified by the National Commission for Certification of Physician Assistants and is a member of the AAPA and DEAPA.

Jessica S. Taylor, FNP
Jessica joined the OA team in 2008. In 2001 she earned her Bachelor of Arts degree in Biology from the University of Maine in Farmington. After working for three years at Maine Medical Center as a nursing assistant, Jessica continued her educational pursuits at the University of Southern Maine where in 2006 she earned her Masters Degree in Nursing and obtained her RN license. Before joining OA as a nurse practitioner Jessica worked in family practice for two years. In her spare time she enjoys skiing and whitewater rafting. Jessica is certified by the American Academy of Nurse Practitioners.

Davoren

Timothy P. Davoren, PT, ATC, Clinical Director
Tim is the Director of OA Physical Therapy Center and clinician at the Physical Therapy Center clinic.  He obtained his Bachelor's Degree in Science from UNE in 1993, is a licensed Physical Therapist and Certified Athletic Trainer. He has fourteen years experience in sports medicine and orthopedics, as well as athletic training for high-school, college, and semi-pro sports.  Tim specializes in athletic knees and shoulders, and sub specializes in treatment of swimmers, runners, and cyclists. He has a focused interest in biomechanics of cyclists and proper bike fit.

Tim has extensive training and education in manual, hands-on therapy through Mulligan, McKenzie, McConnell and others for mobilization and myofascial release. He integrates his hands-on skills with focused sport-specific functional training and applied biomechanics to achieve treatment goals.

Tim believes firmly in injury prevention, and has been staffing Certified Athletic Trainers at high schools throughout Southern Maine for 12+ years.  Since 2000 he has been the Physical Therapy Instructor in the OA/Maine Medical Center Primary Care Sports Medicine Fellowship Training Program.

Tim and his wife Nikki live in Yarmouth with their daughter Kadin and faithful Vizsla Ruger. He is an avid outdoorsman. [physical therapists]  [top]

 

Buzzell

Jared Buzzell, PT
Jared Buzzell is a licensed physical therapist and holds a B.S. degree in physical therapy from the University of Vermont.  He worked in the orthopedic PT department at Dartmouth-Hitchcock Medical Center in New Hampshire before moving to Maine and joining OA in 2000.  He specializes in sports therapy with a special interest in the biomechanics of  the golf swing and how they relate to performance.  He was certified in ‘Back to Golf' in 2005 and attended the first annual Titleist World Golf fitness summit in 2006.  He developed and manages the Golf Performance Program at OA which he does thorough movement and swing assessments with video analysis.

Jared has done extensive course work in various treatment philosophies, including Mulligan, Cyriax, Maitland and Kaltenborn, in addition to numerous hours of functional training and sport specific seminars.  In 2007 he became a Certified Mulligan practitioner (an advanced manual therapy certification) by completing his coursework in the Mulligan concept and passing the credentialing examination.

Jared's passion and enthusiasm for athletics, is evident through his personal involvement in various leagues, ski/snowboarding, golf, and triathlons. This carries over into his treatment philosophy with creating biomechanically sound athletes and promoting athletic wellness/ injury prevention.

Jared resides in  Scarborough, ME with his wife Kate.  The two of them are avid hikers and are members of the AMC White Mountain 4000 footer club. [physical therapists]  [top]

 

Todd Castonguay, PT
Todd received a Bachelor of Science degree in physical therapy from the University of New England in 1997. For the past 10 years he has worked in a general outpatient orthopedic setting for Keystone Rehabilitation Systems in Ephrata and Lebanon, Pennsylvania, where for the last five years he served as director of the Lebanon facility.

Todd has a life-long love of sports, as a participant and a spectator, and he enjoys helping injured athletes return to competition. He has treated a wide variety of musculoskeletal conditions and especially enjoys working with clients who have undergone knee or shoulder surgical procedures.

Doyon

Stacey L. Doyon, OTR/L, CHT
Stacey received her Bachelor of Science degree in Occupational Therapy from the University of New Hampshire in 1984.  Although initially working with neurological and orthopedic injuries, she specializes in hand therapy. She has been with OA since 1989 and in 1991, she received her certification in hand therapy and became a member of the American Society of Hand Therapists (ASHT). Stacey is a past president of ASHT and has been involved on the board since 1998.  Stacey has been a member of the American Occupational Therapy Association since 1985 and a member of the Maine Occupational Therapy Association since moving to Maine in 1986.  Stacey has also been an adjunct professor at the University of New England and has done lecturing here in Maine and across the country in the field of hand therapy. Daniel Frey, DPT, CSCS 
Dan received his Doctor of Physical Therapy degree from the University of New England in 2008. Prior to attending Physical Therapy school, he completed his Bachelor of Science degree in Exercise Science with a minor in Strength and Conditioning at the University of Delaware. While at UD, Dan competed at the Division I level in cross country, indoor track, and outdoor track. Needless to say, he is an avid runner and sports enthusiast.

Throughout his schooling, Dan worked with high school and college level athletes, helping to strength train, condition, and rehabilitate their injuries. He has particular interest in running biomechanics, gait analysis, and overuse running injuries.

Matthew Geary, DPT
Matt received his Bachelor of Science degree in Physical Science from Mesa State College and his Doctorate in Physical Therapy from the University of New England. He has completed course work in foot and gait biomechanics and is passionate about working with runners and triathletes to return to competition. Matt believes in rehabilitation through evidence-based sport specific training.

Matt is an avid runner and triathlete and is involved with local triathlon clubs and races. Before coming to O.A., Matt also worked as a ski patroller in Colorado, Utah and Maine.

Dustin B. Hurd, DPT
Dusty holds a Doctorate Degree in Physical Therapy from the University of New England. He became a licensed physical therapist in the state of Maine in 2008. Dusty’s interests reside in injuries of the musculoskeletal system and he specializes in rehabilitation of injured athletes and post-surgical patients. He utilizes a variety of treatment approaches including Kaltenborn, Mulligan, Paris, and Cyriax for mobilization and orthopedic manual therapy. He combines his biomechanical understanding with functional training for optimal results. He has conducted research concerning lower extremity flexibility in mixed martial artists and has a particular interest in rehabilitation of surfers, skiers and mixed martial artists.

Dusty surfs year-round in Maine, and also enjoys mountain biking, trail running, hiking, sailing, skiing and mixed martial arts. Dusty grew up in central California prior to completing his undergraduate schooling in Hawaii. He currently resides in Cape Elizabeth, with his wife Sarah. [physical therapists]  [top]

 

Mullin

Michael J. Mullin, ATC
Michael is a clinical athletic trainer at OA Physical Therapy Center with more than 14 years of experience. He received his Bachelor of Science degree in Physical and Health Education from Plymouth State College, New Hampshire. In the early 1990's he served on the faculty of New Hampshire Technical College. He is an NATA Certified Athletic Trainer and a registered Physical Therapy Assistant in California.

Michael served as Coordinator of Rehabilitation Services and Head Athletic Trainer at The Stone Clinic in San Francisco, an orthopedic surgery, sports medicine, and rehabilitation facility for 6 years.  He also served as Head Athletic Therapist for Lawrence Pech Dance Company, Smuin Ballets of San Francisco, and The Marin Ballet. He has provided athletic training services for the national champion Old Blues Rugby Club, the U.S. Windsurfing Association National Championships, the US Ski Team, and the World Pro Ski Tour.

He has developed an injury prevention and performance enhancement program called AcCeL which seeks to educate individuals and groups on training and biomechanical awareness.   He is currently a consultant on injury prevention and treatment to Portland Ballet, local and regional high schools and running clubs.

Sports Medicine Author and Consultant: Michael has co-authored a book titled "Comprehensive Approach to Sports Injury Management" and a chapter in "Knee Ligament Rehabilitation" called New Techniques in Cartilage Repair and Replacement".  He has been a sports medicine and fitness consultant for numerous publications such as Shape, Women's Sports & Fitness, Women's Sports Connection, Men's Fitness, Skiing, and Windsurfing magazines. He is a clinic advisor for the American Running & Fitness Association; he also serves on the Editorial Board for Aspen Publications' Sports Medicine and Athletic Training Patient Education Manual. Skolfield

Stanley Skolfield, ATC, CSCS
Stan received his Bachelors degree in Athletic Training at the University of Southern Maine in 1996.  Upon graduation he was hired as the Assistant Athletic Trainer at USM working with the athletic teams, developing strength and conditioning programs, and instructing undergraduate students.

Stan has been involved with the training, nutrition, and rehabilitation of athletes at multiple levels.  He has worked for the Boston Red Sox, providing injury prevention and rehabilitation for in-season teams.  He also implemented off-season performance workouts at the training facility in Fort Myers, Florida.  Upon returning to Maine he worked for HealthSouth and was later hired by OA Physical Therapy Center.  While working in these orthopedic clinics he has incorporated his athletic training, strength & conditioning, and functional orthopedic rehabilitation knowledge into his treatment programs.

Although Stan works with all athletes, he specializes in the injured throwing athlete.  At OA Physical Therapy Center he utilizes the indoor throwing area and video analysis, assisting ballplayers in identifying biomechanical flaws in their throwing motion.  After motion analysis he then focuses on their weaknesses to correct deficits helping the athlete optimize their performance when they return to the field.  Stan is also a speaker for local high schools and colleges, presenting on topics of sports performance program design and implementation.

Skolnekovich

Erica Skolnekovich, PT
Erica received her Bachelor of Science degree in Physical Therapy in 1998 from Daemen College in Buffalo, NY and started her career in Pittsburgh, PA in outpatient rehabilitation.  Upon moving to Maine, she managed a PT clinic in Westbrook offering outpatient orthopedic rehabilitation as well as conditioning programs and treatment to patients undergoing chemotherapy.  For 5 years she actively treated patients and promoted employee wellness, teaching weekly exercise classes for employees.

In 2005 she decided to focus her efforts on treatment of the athletic population and joined OA Physical Therapy Center.  Erica believes in rehabilitation through functional and sport specific training. She has developed and implemented post surgical hip protocols and actively participates in the OA Wellness initiative as a member on the wellness committee.

Erica promotes health and fitness as a certified Jazzercise Instructor, teaching weekly classes in Portland.  She is also a member of the internationally competitive and performing, Royal River Chorus.  She provides weekly active warm ups to develop stamina for performances as well as the promotion of proper singing posture with choreography among members.

Erica and her husband live in Freeport, both enjoy road and mountain biking as well as hiking with their dog. [physical therapists]  [top]

John Wipfler, JD, MBA

Mr. Wipfler joined OA Centers for Orthopedics as chief executive officer in 2006. He was most recently chief executive officer of Eyecare Medical Group, here in Portland. His healthcare-related experience includes positions as the executive director and general counsel of the Maine Health Care Finance Commission, assistant attorney general for the State of Maine representing the Department of Human Services, and healthcare attorney at Pierce Atwood. Mr. Wipfler earned his BA in psychology at Antioch College and both his JD and MBA from Boston College. Mr. Wipfler's interests include spending time with his family, coaching soccer, and travel.
